The patient’s name, address, telephone number, email address and basic medical history are required by Severn Foot Care to provide safe treatment to the patient.  All podiatrists are required to record every appointment they have with their patients and record related treatment notes.  I, Holly Taylor, will collect the above information to enable me to keep a record of each appointment with Severn Foot Care and any subsequent treatment(s).  A photo may be requested to be taken to add to the patient’s treatment notes.  This will be of the foot area only, and you will not be identifiable in the photo. Occasionally, it may be requested to use images for promotional purposes.

 

No person other than I, Holly Taylor, will have access to your data unless you specifically request otherwise.  Your data will not be used by or passed on to any third party unless specifically instructed by you (the patient), except in the event of a medical emergency in which your medical records may be accessed by another healthcare professional (eg, GP or emergency services).

 

Your data is stored using an encrypted digital note-taking service, password-protected and accessed only by me.  Your telephone number/address may be stored on my mobile phone if I need to contact you regarding your appointments.  Your data will be stored for 7 years from the date of your last appointment.  At the end of this period, your data will be securely destroyed.  Holly Taylor is registered with the Information Commissioner’s Office. You may request a copy of the data held about you at any time.
